Senior Product Manager
Form Health
Form Health is a virtual obesity medicine clinic delivering physician-led, evidence-based care for obesity and related cardiometabolic conditions. Obesity affects more than 40% of U.S. adults, and as GLP-1 medications reshape the field, Obesity and Cardiometabolic Medicine is in a period of rapid growth and opportunity. Form Health is at the forefront of that shift, building a clinical model that combines rigorous, personalized medical care with the technology to make it accessible at scale.
Every Form Health patient works closely with a dedicated multidisciplinary Care Team, including board-certified physicians (many ABOM-certified, and endocrinologists), advanced practice providers, and Registered Dietitians who together deliver comprehensive, one-on-one treatment that includes nutrition, behavioral health, physical activity, and FDA-approved medication, if appropriate. Through our mobile app, patients engage in regular video visits along with text messaging, photo journaling, and personalized educational content. We hold ourselves to the highest standards of clinical care and to treating every individual with empathy and respect.
Founded in 2019, Form Health is a venture-backed, mission-driven company partnering with leading employers and health plans to expand access to high-quality, guideline-based obesity and cardiometabolic care nationwide. We're deeply invested in our core value of putting patients first, and equally committed to building a culture where every employee is valued and we learn and grow together.
About the Role:
The Care Delivery team builds the tools that let our clinical team run a high-performing virtual practice — from prescribing and prior authorizations to the day-to-day workflows clinicians rely on to do their best work with patients. This role is highly visible within Form Health: the tools you ship directly shape how our clinical team practices and, downstream, how our patients experience care. We're looking for someone who's genuinely energized by hard, regulated, multi-stakeholder problems — with clinical, patient, payer, and operational constraints — as well as someone who uses AI as a real part of supercharging their work, rather than a buzzword on a resume.
This role will report to our Lead Product Manager for Care Delivery, with significant ownership from day one and plenty of room to help shape how the practice scales as the team grows. This role is fully remote and can be located anywhere in the U.S. (Note: while not required, it is beneficial to be in or close to the Eastern Time zone given frequent collaboration with our Boston-based clinical and operations leaders)
What You Will Do:
- Deeply understand Form Health's clinical and business needs: identify and prioritize jobs-to-be-done using research and user feedback
- Own key care delivery capabilities end-to-end — e.g., prescribing, prior authorizations, virtual visit workflows
- Partner with clinical leadership to triage and prioritize incoming product requests, balancing clinical needs against broader roadmap priorities
- Translate business and user needs into clear product requirements, including user stories, use cases, and functional specs
- Partner with engineering, design, operations, and clinical teams to ship best-in-class tools for our clinical practice
- Track key program and product metrics for your area, and report on progress and health to Form Health's leadership team
- Facilitate sprint planning, standups, backlog grooming, and retrospectives to keep the team aligned and shipping
- Partner with clinical education and enablement to roll out new features smoothly — change management, communications, and training
- Identify where AI and automation can responsibly reduce clinical or administrative burden (e.g., documentation, prior-auth workflows, care coordination), and partner with engineering to evaluate and ship those capabilities thoughtfully
About You:
- 5+ years building enterprise experiences with complex workflows
- Comfortable diving into technical problems — APIs, workflows spanning homegrown and third-party systems
- Clear communicator cross-functionally in a remote-first environment
- A track record that includes:
- Evaluating third-party vendors and driving coordination across multiple systems
- Defining where a product should go, rallying a team behind it, and delivering results iteratively
- Owning a product area end-to-end — strategy and prioritization through rollout and adoption
- Fluent in using AI as part of your own PM craft — drafting specs, synthesizing research, prototyping — and curious about where AI can responsibly augment clinical operations
- Proactive ownership over your area of responsibility, with a strong sense of accountability and an execution-oriented mindset — comfortable applying structure to ambiguous situations in a fast-growing startup
- Genuinely excited to make compassionate, evidence-based healthcare accessible — and energized, not deterred, by the complexity of a regulated, high-stakes domain
Preferred:
- Understanding of healthcare, virtual care delivery, or health insurance
- Experience shipping AI-powered features, ideally within a tech-enabled services organization
- Prior experience in a startup and/or high growth environment
More about Form Health’s benefits:
- Competitive salary and equity in a high growth start-up
- Comprehensive health benefits (medical, dental, vision)
- 401k program
- Flexible work schedules and paid time off
- Paid parental leave
Form Health’s commitment to building a diverse, equitable, and inclusive work environment:
Form Health is committed to creating a culture and environment that celebrates diversity and inclusion, while fostering safety and belonging. This extends from our remote patient care to our corporate offices and everywhere in between. We are looking for team members who want to help us further our Diversity, Equity, and Inclusion (DEI) efforts and who share our attitudes for creating an inclusive, safe, and positive work environment.

Spark
About Spark Advisors
We're building healthcare tech for a system that desperately needs it.
Each year, millions of Americans deal with confusing changes to their Medicare plans that impact their prescriptions, access to care, and pocketbooks. These aren’t just inconveniences – they are potentially life-altering changes that leave seniors medically and financially vulnerable.
Independent Medicare advisors play a crucial role in guiding seniors through this complexity and helping them find the right coverage for their needs. But their ability to be effective healthcare advocates has long been hamstrung by broken tools and outdated systems.
Spark is fixing that. We’re the fastest-growing Medicare platform in the country, combining AI, an industry-leading CRM, and client services to transform how 10,000+ brokers acquire, enroll, and support clients in their local communities.
Join a talent-dense team from Square, Ramp, Yext, Oscar, and Cedar — backed by Primary Ventures and Viewpoint Ventures — that is serious about building technology to expand access to quality healthcare. We offer remote work, sabbaticals, company retreats, and other generous benefits that earned us recognition as one of Inc. Magazine’s Best Workplaces of 2025
Healthcare is overdue for innovation. Let’s redefine what its future looks like — together.
Summary
We're looking for a Senior Product Manager to own a critical internal platform that powers how Spark manages carrier contracting, commissions, and agency operations. This isn't a simple internal tool, it's a purpose-built product that encodes the operational intelligence behind Spark's ability to scale.
You'll work closely with engineering, design, and the operations team who are the platform's primary users, translating complex regulatory and operational workflows into a system that's intuitive, reliable, and built to grow with the business. This role sits across execution and discovery: you'll need to be energized by shipping intentional, user-centric workflows while staying curious about the underlying systems and data that make those workflows possible.
We're looking for someone comfortable operating independently and partnering with senior stakeholders, including C-level leadership at a fast-moving Series B startup. Ideally you're a strong generalist with prior experience building internal tools or systems of record, and you bring deep workflow intuition and regulatory awareness to the role.
What You'll Do
- Own key product areas related to scaling operations
- Collaborate with engineering and design to define product specs, ship high-quality features, and drive adoption
- Maintain a tight feedback loop with the ops team, treating them as a first-class product constituency with their own personas, workflows, and success metrics
- Conduct user interviews, analyze product usage, and run experiments to identify high-impact opportunities
- Act as a subject matter expert in your domain and ensure alignment with broader product strategy
- Bring clarity and structure to ambiguous problems and workstreams
What We're Looking For
- You have a track record for managing a successful product domain, ideally in B2B SaaS or platform products
- You write clear product specs and have strong instincts for when to be scrappy vs when to be rigorous
- You're comfortable navigating cross-functional work and can clearly communicate decisions and tradeoffs
- You use data to inform decisions, whether through dashboards, SQL, or close partnership with data teams
- You've seen products through multiple launch cycles and know how to set goals, measure impact, and iterate
- You thrive in ambiguous, fast-paced environments and take ownership without waiting to be asked
- You're deeply empathetic toward all the different internal and external stakeholders you work with
Compensation
Our salary ranges are based on paying competitively for our company’s size and industry, and are one part of the total compensation package that also includes equity, benefits, and other opportunities at Spark. In accordance with New York City, Colorado, California, and other applicable laws, Spark is required to provide a reasonable estimate of the compensation range for this role. Individual pay decisions are ultimately based on a number of factors, including qualifications for the role, experience level, skillset, geography, and balancing internal equity. A reasonable estimate of the current salary range is listed below. We expect most candidates to fall in the middle of the range. We also believe that your personal needs and preferences should be taken into consideration, so we allow some choice between equity and cash.
Why you should join our team
By joining Spark, you will get in on the ground floor of a fast-moving, well-funded, and mission-driven startup where you will have a profound impact on the brokers and beneficiaries we serve. And you'll learn, grow, be challenged, and have fun with your team while doing it.
We strive to help you and your family thrive. We're committed to supporting your happiness, healthiness, and overall well-being by providing a comprehensive benefits program. In addition to your base salary, we also offer:
- Equity compensation
- Health care, including dental and vision through our PEO Sequoia
- Flexible work location; co-working available
- 401k
- Paid Time Off
- Monthly Remote Work Stipend (help cover costs of home-office needs)
- Paid Parental Leave
- Up to 14 weeks for birthing parents
- Up to 8 weeks for non-birth parents
- 11 paid holidays
- 2 week sabbatical at 5 years of employment
- Wellbeing Perks through SpringHealth, OneMedical, PerkSpot, and SoFi

We need diverse perspectives to reflect the diversity of our problems and the population we serve. We look to hire people from a variety of backgrounds, including, but not limited to, race, age, sexual orientation, gender identity and expression, national origin, religion, disability, and veteran status.
