BAYADA Home Health Care is seeking a compassionate and motivated Physical Therapist (PT) to join our Ocean County Senior Living Team , a Medicare-certified home health practice serving adult and geriatric clients throughout Ocean County, NJ.
In this role, you'll provide individualized therapy services to clients residing in assisted living communities, independent living communities, senior congregate housing, 55+ communities, and intermediate care facilities for individuals with intellectual disabilities. Responsibilities
Provide physical therapy evaluations and develop individualized treatment plans for adult and geriatric clients.
Assess mobility, strength, balance, endurance, gait, transfers, range of motion, and overall functional status.
Implement evidence-based interventions including therapeutic exercise, gait training, balance training, fall prevention, and functional mobility programs.
Utilize integumentary protective interventions to prevent wound formation and promote healing when appropriate.
Collaborate with nurses, therapists, physicians, caregivers, and other healthcare professionals to coordinate patient-centered care.
Continually assess patient progress and modify treatment plans to maximize functional outcomes.
Educate patients, family members, and caregivers on exercises, safety strategies, adaptive techniques, and disease management.
Complete timely and accurate documentation utilizing electronic medical records and point-of-care technology.
Qualifications
Current New Jersey Physical Therapist (PT) license.
Minimum of one year of recent experience as a Physical Therapist.
Graduate of an accredited Physical Therapy program approved by the American Physical Therapy Association (APTA).
Home health experience preferred but not required.
Strong assessment, clinical reasoning, and communication skills.
Ability to work independently and effectively manage time and priorities.
Proficiency with electronic medical records and technology.

Compensation
Base Pay, depending upon qualifications, status, and subject to negotiation: $70.00–$75.00 per point ($109,200 - $117,00 annual earning potential)
Compensation is based on qualifications, experience, productivity, and employment status.

What You'll Do
Perform comprehensive occupational therapy evaluations and develop individualized treatment plans for clients in senior living communities and group home settings.
Utilize evidence-based interventions to maximize safety, independence, and quality of life.
Collaborate with home health clinicians to ensure seamless transitions of care and achieve client-centered goals.
Educate patients, families, and caregivers on therapeutic techniques, adaptive equipment, and home safety recommendations.
Deliver skilled occupational therapy treatments and complete timely documentation through our electronic medical record platform.
Assess functional abilities related to activities of daily living (ADLs), mobility, cognition, and environmental safety.
Participate in discharge planning and ongoing care coordination to promote optimal patient outcomes.
Qualifications
Current New Jersey Occupational Therapist (OT) license
Minimum of one year of recent experience as an Occupational Therapist
