Are you passionate about supporting Pasifika people and creating positive change in your community?
Lifewise is seeking a Pasifika Disability Outcomes Coach to empower Pasifika disabled people, their aiga, families, and carers to achieve their goals and live good lives. In this rewarding role, you'll build trusted community relationships, support people to access and self-manage disability funding, and champion the Enabling Good Lives principles of choice, control, and inclusion.
What You'll Do
Build relationships with Pasifika communities, churches, and organisations.
Support Pasifika disabled people and their families and carers to access Individualised Funding (IF) and disability supports.
Coach people to confidently manage their funding and achieve their goals.
Promote Lifewise services and increase community awareness and referrals.
Represent Lifewise at community events and fono.
What You'll Bring
Strong knowledge of Pasifika cultures, values, and communities.
Fluency in at least one Pacific language.
Experience in disability, health, social services, or community support.
Excellent communication and relationship-building skills.
A passion for empowering people and strengthening communities.
Full NZ Driver Licence.

The Details
Based in Grafton, Auckland
Permanent Full-Time (40 hours per week)
Monday to Friday, with some flexibility for community events outside normal hours.

About the Role | Mō te tūranga
This is more than a case management role.
As a Disability Outcomes Coach, you'll walk alongside people who self-manage their disability funding and employ their own support staff. You'll simplify complex systems, build confidence, and empower individuals and whānau to take control of their support journey.
You'll bring the Enabling Good Lives principles to life, helping people turn aspirations into achievable, measurable outcomes.
If you bring lived experience of disability, either personally or within your whānau, or have strong advocacy experience within the disability sector, we would genuinely value your perspective.
What You'll Do
Manage a caseload of Individualised Funding (IF) and Younger Persons with Disabilities (YPD) clients and whānau
Coach individuals to confidently self-manage their funding
Support the development and review of personal outcome plans
Provide clear guidance on purchasing rules and eligible claims under Whaikaha – Ministry of Disabled People
Celebrate progress, milestones, and achievements alongside the people you support
Maintain accurate records that reflect each person's journey and outcomes achieved
What You'll Bring | Ō Pūkenga
Experience in disability, health, community, or client-focused support services
A strong belief in self-determination, choice, and empowerment
Excellent communication and active listening skills
The ability to explain complex information clearly and confidently
Strong computer and database skills
A collaborative approach with the ability to work autonomously
A full NZ Driver Licence
The legal right to work in New Zealand
