BAYADA Home Health Care is seeking an experienced Occupational Therapist (OT) to join our Essex County Visits team based in Livingston, NJ. Responsibilities:
Provide skilled occupational therapy services through home visits within designated territories.
Perform comprehensive evaluations of patients' functional abilities, including strength, endurance, coordination, cognition, communication, and activities of daily living (ADLs and IADLs).
Develop individualized treatment plans to maximize independence, safety, and functional outcomes.
Assess home environments for safety concerns and recommend modifications or adaptive equipment to support independent living.
Identify and recommend assistive devices and environmental adaptations to improve patient function and quality of life.
Collaborate with nursing and interdisciplinary team members to coordinate comprehensive patient care.
Educate patients, caregivers, and family members on therapeutic techniques and strategies to promote safety and independence.
Continuously assess patient progress and adjust treatment plans as appropriate.
Accurately document evaluations, interventions, and patient progress using electronic medical records and tablet technology.
Qualifications:
Current New Jersey Occupational Therapist (OT) license.
Minimum of one year of recent Occupational Therapy experience.
Prior home health experience preferred but not required.
Graduate of an accredited Occupational Therapy program.
Strong assessment, communication, organizational, and problem-solving skills.
Ability to work independently and effectively manage time.
Electronic medical record (EMR) experience preferred.

Additional Information:
Base Pay, depending upon qualifications, status, and subject to negotiation:
$75.00-$80.00 per point ($117,000-$124,800 annual earning potential)

Provide skilled physical therapy services through home visits within designated territories.
Perform comprehensive evaluations, including assessments of mobility, strength, balance, gait, endurance, transfers, safety, and functional abilities.
Develop individualized treatment plans based on patient needs and physician orders.
Implement evidence-based therapeutic interventions, including therapeutic exercise, gait training, balance activities, and mobility training.
Provide integumentary and wound prevention interventions as clinically appropriate.
Collaborate with nursing and interdisciplinary team members to coordinate patient care.
Educate patients, caregivers, and family members to promote safety, independence, and successful recovery at home.
Continuously assess patient progress and modify treatment plans as needed.
Accurately document evaluations, interventions, and patient progress using electronic medical records and tablet technology.
Qualifications:
