This per diem position is to service clients within senior living communities– it’s home health without the typical home health commute! Registered Nurse (RN) Responsibilities: Follow a designated care plan in accordance with patient's needs Make home visits to clients in designated geographic territories Perform assigned duties, including administration of medication, wound care, treatments, and procedures Monitor clients' conditions; reporting changes to Clinical or Client Services Manager Follow up with, execute, and properly document doctors' orders Perform client assessments as necessary Case management and coordination Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet Qualifications for a Registered Nurse (RN): A current license as a Registered Nurse in Maryland. A minimum of one year of recent, verifiable clinical (medical/surgical) experience. Prior home care experience strongly preferred, but not required. Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma. Ability to work independently and manage time effectively. Strong interpersonal skills. Solid computer skills; prior experience with electronic medical records (EMR) preferred. Ability to travel to cases as assigned. BAYADA Home Health Care is currently seeking a Physical Therapist Assistant for a Full Time opportunity throughout Towson and Pikesville, Maryland (Baltimore County). This full-time position is to service clients within senior living communities– it’s home health without the typical home health commute! Must be available Monday thru Friday between 8:30am-5:00pm. As a home care Physical Therapist Assistant (PTA) you will assist clients in attaining or maintaining optimal physical well being by providing treatment, tests, client education and appropriate interventions as delegated by a licensed Physical Therapist. Each Physical Therapist Assistant (PTA) must: Have current licensure or certification in the state of Maryland Have graduated from a two-year college level program approved by the American Physical Therapy Association, as indicated by school transcript or diploma, or Have one year of appropriate experience as an Physical Therapist Assistant (PTA) and a satisfactory grade on a proficiency exam conducted, approved or sponsored by the U.S. Public Health Service when initial qualification occurred before December 31, 1977 Have at least one year’s work experience under the supervision of a qualified Physical Therapist.
