Asa Physiotherapist,youareresponsible forthe assessment of neuromuscular,musculoskeletaland cardiorespiratory systemsand the treatment, rehabilitation and preventionof aninjury or pain.The goal isto develop,maintain,rehabilitate,or augment function and promote mobility. Care will take place within a residential or institutional setting with referrals generated by a variety of sources.Client groups receiving treatment may includepediatrics for in home or school services and/oradults with medical/health concerns.

Whatyou’lldo:
Conduct initial client assessments, identification of problems, and establishment of goals and treatment plans
Develop client-centered plans with clients, inter-professionalteamand other stakeholders
Engage clients using a collaborative approach, build rapport and trust, and achieve a mutual understanding and agreement related to the services to be provided
Maintain professional client records related to client encounters and plans
Work collaboratively with a multi-disciplinary team consisting of government agencies, healthcare workers, and other community resources
Whatyou bring:
Bachelor's degree in Physiotherapy
Registered and in good standing with the College of Physiotherapists of Ontario
Willingness to travel to and from client’s residences within a designated area
Validdriver licenseand access to a vehicleto travel to and from patient’s residences within a geographical area

In this community nursing role, you as a Registered Practical Nurse (RPN) will have scheduled visits within the community to care for patientsin various settings with the support of RNs for more complex patients. You will work with patients of all ages with differing complexity of care needs and play a key role in helping patientsremaincomfortable in their homes for as long as possible. Whatyou’lldo:
Participate in scheduled visits in the community to look after patient needs within your scope of practice
Provide ethical and safe nursing care
Useappropriate assessmenttools/techniques to assess patients’ individual needs
Execute and continuouslymonitorcare plans and adjust asrequired
Engage patients using a collaborative approach by building rapport and trust
Maintain and update professional patient records related to patient encounters and plans
Whatyou bring:
Registration with the CNO as a RegisteredPracticalNurse in good standing
Valid driver’s license and access to a vehicle isrequired
Experience in delivering community care is an asset
Reliability and flexibility
Willingness to travel to and frompatientresidences within a designated area
Experience working with the vulnerable population

Required on-call coverage, including after hours to ensure patient safety and to meet patient care needs
