Sun River Heath is a Federally Qualified, Non-Profit Health Center serving communities in Suffolk, Rockland, Orange, Dutchess, Ulster, Sullivan, Columbia and Westchester County. Sun River Health is currently seeking a Full- Time Psychiatrist at The HUB Health Center site, located in the Bronx, NY.
The psychiatrist provides psychiatric services within the Article 31 of Sun River Health. This individual provides quality care to patients within the practice. This individual coordinates with the SVP of Behavioral Health and other Article 31 professionals in order to uphold the standards of psychiatric interventions in accordance Sun River Health clinical standards of practices and in conjunction with federal, state and local regulations.
Essential Duties and Responsibilities:
Provides direct patient psychiatric care according to professional guidelines and best practices.
Maintains collaborative agreements with the psychiatric nurse practitioners.
Participates in the development of clinical systems that assures the provision of care for all clients in accordance with federal, state and local regulations.
Participates in developing plans of care for clients in collaboration with the multidisciplinary treatment team.
Maintains liaison with other departments/disciplines relating to the provision of psychiatric care.
Consults with Administration, Nursing, Social Work and Substance Abuse Services to evaluate the facility's ability to meet the psycho-social needs of the client.
Provides for appropriate psychiatric educational programs for mental health and medical staff and participates in in-service training within organizations.
Provides patient advocacy.
Participates in continuing education programs appropriate to the practice.
Maintains confidential case records, medical charts and files pertaining to the resident/client.
Embraces and exemplifies the Core Values of Brightpoint Health.
Is able to communicate and provide care in Spanish preferred.
Participates in programs, meetings, conferences that satisfy requirements for continuing medical education, enhance the Department of Mental Health and/or further the mission.

Doctorate Degree (including M.D., D.O., D.D., PhD., etc.)
License to practice Medicine in New York State

SUMMARY OF POSITION:
The Health Home Care Manager provides care coordination and support to clients with chronic medical and behavioral health conditions that are also impacted by social determinants of health. Assists clients with navigating social service, community, and healthcare systems.
ESSENTIAL FUNCTIONS:
Completes comprehensive assessments within the required timeframes. Maintains detailed, accurate and timely case notes. Conducts intakes as needed
Facilitates enrollment in Benefit and Entitlement programs. Develops linkages and refers patients for additional service supports
Provides timely and appropriate follow up on newly referred clients
Provides Health Home Care Management services at community-based locations and within the Sun River health centers
Facilitates periodic case record reviews and case conferences with all providers serving the client
Provides linkage, coordination with, referral to and follow-up with appropriate ongoing service providers, including mental health and medical specialists
Case conferences with interdisciplinary team including but not limited to PCP, substance abuse treatment team, residential, hospital discharge planners, etc., to coordinate care delivery between all linked providers and client
Conducts field work to meet their clients in the community
Maintains data and case records as required and prepares necessary reports
Develops, coordinates and integrates a coordinated care plan in cooperation with the client, the client's family, and/or the other providers serving the patient. Updates plan at specified intervals, and as needed based on changes in client's condition / circumstances
Performs and maintains effective care management for a caseload of clients, as assigned, from assessment to discharge
Tracks/ monitors client progress and produces/maintains detailed, accurate and timely case notes
Maintains updated case records through health home EMR, and coordinates effective electronic communication throughout all provider databases, as needed. Maintains case records in accordance with health home policies/procedures, agency standards and regulatory requirements
Participates and consults with team supervisor in case conferences, staff meetings, and discharge planning meetings to determine if client requires an alternate level of care or is appropriate for discharge
EDUCATION/EXPERIENCE:
Bachelor's degree preferred in Health or Human Services related field with 2 years of related work experience. High School Diploma/GED required.

Hydraulics experience is highly preferred
Engine and transmission maintenance and repairs

Chassis component repairs and maintenance including air brakes system, suspension, drivetrain and steering components.

Interstate Waste Services – IWS – is hiring a Diesel Mechanic for our Goshen Transfer Station location. This is an opportunity to work hard, make good money, and grow a successful career as a Diesel Mechanic.
Our Diesel Mechanics perform inspection, diagnosis, preventative maintenance, and repair of electrical, hydraulic, suspension, brake and air systems on vehicles and equipment, including diesel heavy trucks and engines. This specific role with deal primarily with our tractor trailer division. Utilizing diagnostic software, the Mechanic is responsible for interpreting failure modes and initiate or assign necessary repairs. At times, the Mechanic may be on call, and required to assist with emergency breakdowns.

Experience working on heavy duty diesel equipment OR training from a recognized trade school is required.
