Job Summary:
Provides temporary practice, pending results of the licensing examination for a period not to exceed 90 calendar days after the date of graduation. This temporary practice privilege automatically ceases upon notification that the graduate nurse has failed the licensure examination or once the 90 calendar days after the date of graduation has past and required licensing is still pending.
Provides holistic, quality and cost-effective nursing care for patients/families and/or others for a patient population within a unit, service or other primary area of practice with an awareness of the episode of care. Applies the nursing process within the context of the organization's Framework for Professional Practice. Practices under the supervision of a licensed Registered Nurse. Reports to Unit Manager.
Qualifications
Graduate of an accredited Nursing program. Minimum Associate degree, Bachelor’s preferred. Associate degree hires must obtain a BSN within 6 years of hire date

Serves as a bridge between the SSO, healthcare team and the patient and/or caregivers.
• Communicate and coordinate referrals and patient care with the onsite TCCs and/or clinical areas to provide seamless care to patients. Acts as agency’s point of contact and liaison for other agency departments.
• Locate patients that transfer to hospital and communicate with clinical teams and TCCs.
• Receive and resolve inquiries for referral data/items required for coding and billing.
• Effective communication skills, self-directed, with a spirit of team support and success, curiosity and ownership, flexibility and a consistent demonstration of H3W Leadership Behaviors and modeling
