Illinois Bone & Joint is looking for a Full Time Physical Therapist who would be a part of this multidisciplinary team providing collaborative care addressing adult and pediatric obesity. In this role, you will work alongside dieticians, health coaches and physicians to provide a comprehensive care plan to optimize a patient’s metabolic health.

Essential Job Functions
Exceptional patient care Demonstrate empathy and understanding of patients ability and function that will support a behavioral change in patients resistance to movement and exercise Familiarity with the NEAT philosophy for exercise training Develop group exercise programs for participants in the OrthoHealth Program Have an interest in evidence based education and exercise plans that can be tailored to fit patients individualized needs Interest in Nutrition, sleep and health coaching as they impact metabolic health Interprets physician orders, performs initial evaluations and establishes treatment plans and functional goals Responsible for upgrading and amending the treatment goals as necessary during the episode of care Written documentation on patient status to physicians per compliance guidelines Determine appropriate discharge timeline and appropriate follow up Compliance with daily paperwork. PTs are responsible for documenting all treatment in patient charts in a timely manner (initial evals, daily notes, progress notes, discharges, HEP) Stays current with billing policies and demonstrates ethical billing practices Maintains active state license by earning appropriate CEUs through outside courses and IBJI sponsored educational opportunities Develops positive relationships with the physician teams at IBJI

Job Summary
The Physical Therapist is under the general direction of the Facility Manager. The main focus of the position is on providing superior outpatient rehabilitation services through a collaborative care model with our physicians in order to optimize performance and enhance the quality of function for our patients.
Physical Therapist Responsibilities

PT/PTAs/ATCs are responsible for documenting all treatment in patient charts in a timely manner (initial evals, daily notes, progress notes, discharges, HEP) Stays current with billing policies and demonstrates ethical billing practices Maintains active state license by earning appropriate CEUs through outside courses and IBJI sponsored educational opportunities Develops positive relationships with the physician teams at IBJI Takes an active role in the patient medial team: office visits with physicians, contact with case managers, etc. Participates in programs sponsored by IBJI: Mentoring, clinical educator, special interest group, community outreach
