Metcalfe Family Medical Center is seeking a skilled and caring CMA, LPN, or RN to join our team and help deliver exceptional patient-centered care. The CMA/LPN/RN plays a key role in supporting providers and patients across all aspects of primary care. This role involves patient assessment, assisting in clinical tests and procedures, and ensuring that care is delivered efficiently and compassionately. The ideal candidate is a team player who thrives in a collaborative, community-focused environment.
Job Functions/Responsibilities:
Consult with and keep supervisor informed of clinical activities, needs, and problems related to patient care
Complies with established patient care standards.
Maintains established clinical policies, procedures, objectives, quality assurance, safety, environmental and infection control.
Maintains a clean, orderly and safe environment.
Cooperates with other personnel to achieve department objectives and maintain good employee interdepartmental and public relations.
Greets patient and escort them to the exam room.
Takes and records vital and patient medical information in chart.
Prepares patient room for exam.
Assist Provider in procedures as necessary.
Completes ordered lab procedures.
Administer IM, Sub Q injections to include immunizations.
Wound care and suture removal as directed by Provider.
Communicate results of all procedures and tests to Provider.
Discharge patient to receptionist.
Cleans and disinfects exam rooms after use.
Restock supplies, linens, equipment, and checks for defects and outdated.
Maintains professional growth and development through seminars and educational sessions are required.
Other duties as assigned.
Qualifications
Minimum Qualifications:
Active license or certification
CPR Certification.
Experience preferred.
Licensure/Certification within good standing.
Maintain required continuing education courses.
Schedule Requirements:
Full-time position, 4-5 days per week, 100% clinic-based, in person work required.
Work schedules are set at the discretion of CFMC and may be adjusted at any time based on the operational needs of the organization.
Compensation: Negotiable based on experience

The Registered Nurse Case Manager (RN) will be responsible for providing care and services for a group of patients and visiting patients where they reside, including private homes, Skilled Nursing Facilities, and assisted living facilities. This position is a contributing interdisciplinary team member, providing expertise regarding psych-social assessments, interventions, and knowledge of community sources.
Qualifications
Current Registered Nurse (RN) licensure (in good standing) in the state of Georgia
Works well within a team environment to collaborate daily with our interdisciplinary team
Work closely with patients on a 1-1 level but get strong support from a patient care team.
Can work independently and on your own from a day-to-day basis with activities which include: phone, email and written communication, meetings with a variety of healthcare professionals, and an understanding of hospice rules and regulations.
Time management skills to manage visits, documentation, meetings (i.e.: to scheduled visits, attend required meetings, respond to urgent needs by prioritizing and triaging multiple concerns)
Ability to learn an electronic medical records system.
Must be willing to drive with reliable transportation, valid driver’s license, and auto insurance, to be able to travel across the service area
