Apply now so we can make a lasting impact together! Position Summary The Caregiver Program Specialist provides support, education, resource navigation, and outreach services for family caregivers. This position facilitates support groups and educational programs, connects participants with community resources, develops partnerships with service providers, and promotes positive outcomes for caregivers and families through a strengths-based, person-centered approach. This role is ideal for candidates with backgrounds in social work, gerontology, counseling, family services, community outreach, or related human services fields. General Responsibilities Utilize social work skills to implement community-based educational programs for caregivers or relatives raising children Utilize Case Management training and experience in gerontology and psychotherapy to support therapeutic outcomes. Implement programming under the direction of the Manager, including support groups, educational opportunities, outreach, and information/referral for caregivers. Using social work, facilitate caregiver support groups, provide resource navigation and referrals, and connect caregivers to services that strengthen family stability, well-being, and quality of life. Implement special projects as assigned by the Manager. Adhere to guidelines set forth in funder’s bylaws and standards. Maintain secure client files and documentation in compliance with agency and funder standards. Attend outreach events to recruit clients and share information on programming. Participate in agency, funder, and community meetings and trainings. Complete recruitment activities for caregiver services and maintain distribution lists for advertising. Maintain cooperative relationships with service providers and community organizations. Act as an agency ambassador in communities served, attending community groups or events as required. Work cooperatively to ensure effective program outcomes and outputs for caregivers. Other duties as assigned within the guidelines of this position. Determine eligibility for Illinois Department on Aging Waiver Program (Community Care Program). 3. Conduct comprehensive needs assessments and determine eligibility for services. 4. Establish written care or treatment plans with clients and families for maximum independence. 5. Active intervention and advocacy to access necessary services from various programs. 6. Complete monitoring, follow-up, and proper termination of cases. 7. Ensure the completion of Intensive Casework with each completed comprehensive assessment. 8. Complete critical incident reports in accordance with Illinois Department on Aging Standards. 9. Maintain confidential case records and meet documentation standards. 10. Accurate completion of all required reporting and documentation. 11. Maintain cooperative relationships with service providers and community organizations. 12. Conduct educational presentations for professional and community groups. 13. Attend training, staff, and team meetings, and act as an Agency ambassador in the community. 14. Other duties as assigned within the guidelines of this position. Active intervention, advocacy and social work to access necessary services from various programs and complete any necessary referral forms. Complete proper termination of cases. Maintain confidential case records and meet documentation standards. Accurate completion of all required reporting and documentation. As a Case Manager, maintain cooperative relationships with service providers and community organizations, including regular contact with Managed Care Organizations. Submit accurate files to billing in a timely manner. Attend training, staff, and team meetings, and act as an Agency ambassador in the community. Other duties as assigned within the guidelines of this position. Represent the agency and promote the program through community engagement. Assist in recognizing volunteers and maintaining accurate program records.
